Authority and provenance
Official final reports are the authority for facts and findings. Air France uses the BEA report and its recorder chronology. Air Canada uses the TSB final report, including its built-in appendices. Warsaw uses a third-party rendering of Peter Ladkin's transcription of the Polish commission report. It omits figures and appendices and has not been compared with an original scan. Its review status is lower.
Source URLs, retrieval dates, versions, byte sizes, SHA-256 hashes and individual PDF page boundaries are retained locally. Public source panels show the provenance and hashes. PDFs and complete extracted reports are not offered as downloads from this site.
Search and extraction
A repeatable local process downloads reports, checks that the files are PDFs and extracts text page by page with Poppler. Search uses literal words and phrases in that text. It returns short original-text excerpts, highlights matching terms and links to the source page. There is no chatbot or generated answer in search.
Results are limited to six matching pages and 40 words per excerpt. Line wraps and spacing are normalised. Search can miss figures, image-only text or phrases broken by hyphenation. It covers the report text, not every investigation record. Air France's recorder appendix is in French; Warsaw's original appendices are not indexed.
Printed pages and PDF pages
Printed page numbers refer to the report itself. PDF page numbers count every PDF sheet, starting at one, including covers. Zero-based indexes start at zero and are shown separately. Links use the one-based PDF page number in #page=. A browser's PDF viewer may ignore that fragment; use the displayed PDF page in that case.
The Warsaw rendering sometimes places parts of two original report pages on one PDF sheet. Timeline citations identify both the original reference and the PDF rendering page. Search results ask you to inspect the preserved original page footer. No missing original page is silently inferred.
AI and review status
AI assisted source location, drafting, text extraction and comparison of claims with report passages. Published claim references were checked against the extracted source pages; selected pages were visually inspected. This is a source check performed with AI assistance, not independent expert review, community review or certification.
Investigation reports themselves can include uncertainty and internal inconsistencies. We retain words such as likely and possible when material. We exclude the inconsistent middle-marker time from the Warsaw timeline. A safety investigation does not determine civil or criminal liability.
Classification rules
Circumstances describe the setting. Initiating events describe the start of a sequence. System behaviour describes an aircraft response. Crew actions describe what the crew did or did not do. Official contributing factors are reserved for the report's conclusions or equivalent explanatory factors. Labels may overlap without becoming a single main cause.
Related links are limited to these three dossiers and require the same supported classification. For example, Automation and sensing includes different mechanisms in the three reports. The label does not claim they share the same failure.
Reuse and corrections
We link to original documents and use short quotations in source-based explanation. BEA terms restrict wider commercial or promotional reuse. No licence for full redistribution of the Warsaw transcription was established. TSB excerpts are attributed non-commercial reproductions from the linked official version, without TSB endorsement. Figures, manual pages and cockpit transcripts are not republished.
The correction form saves a private editorial submission with a reference number. It does not publish comments or automatically change the site. Corrections are retained for up to 90 days. No name or email is required and no reply is promised.
Privacy
No advertising, visitor analytics or tracking cookies are added by this site. The host may process normal request information. The correction route temporarily stores a daily hash of the connecting IP for submission limiting, not the raw IP. Avoid personal or sensitive information in corrections.
